Bottle instructions for daycare
How to prepare this baby’s bottles. Fill it in and hand it over.
The basics
- Baby’s name
- Date of birth
- Formula brand and stage
- Scoops per bottle, and to how much water
- Usual amount per bottle
- Usual number of bottles per day
- Water we use (tap, filtered, bottled)
Allergies, medication and anything else
- Allergies or diagnosed conditions
- Notes
House rules that are not negotiable
- Use only the formula sent with this card. Do not substitute a brand.
- Follow the scoop-to-water ratio above exactly. Scoop sizes differ between brands.
- A bottle the baby has drunk from goes in the bin after 1 hour. Do not refrigerate it.
- Prepared bottles keep 2 hours at room temperature, 24 hours refrigerated.
- Never warm a bottle in a microwave. It heats unevenly and the bottle feels cooler than the milk.
