Yes. Combining already-prepared formula and expressed breast milk in one bottle is accepted practice, and for a lot of families it is the difference between a feed that happens and a feed that turns into a fight. But there is one rule that is not negotiable, and one consequence that catches almost everybody out.
The rule: water first, then combine
Prepare the formula with water, exactly to the manufacturer's instructions. Then add the breast milk. Never use breast milk in place of the water to reconstitute powder.
The reason is concentration. A scoop of powdered formula is designed to dissolve into a specific volume of water, which contributes nothing but liquid. Breast milk is not water: it already carries its own protein, minerals and sugars. Reconstitute the powder with milk instead and you get the full solute load of the formula stacked on top of the milk's own, in the same volume of fluid.
That produces a feed more concentrated than either component was meant to be. Over-concentrated formula is the specific preparation error associated with dehydration in infants, because a baby's kidneys have to shed the excess solute using water the baby needs. It also wastes a startling amount of expressed milk, since you would be using milk to do a job water does for free.

The consequence: the bottle inherits formula's clock
This is the part most parents do not see coming. Breast milk and prepared formula are on different storage clocks, and formula's is much shorter.

Once the two are in the same bottle, the formula rule governs, because you cannot apply the more generous limit to a liquid that now contains the stricter one. So a bottle of 3 oz expressed milk plus 2 oz formula is a 2-hour bottle, not a 4-hour one — and if the baby drinks half and stops, the rest goes in the bin within the hour.

Why many combo-feeding families keep them separate
Given the above, the common pattern is not mixing at all: breast milk first, usually at the breast, then formula after as a top-up in its own bottle.
That sequence does three things at once. Nothing gets thrown away that did not have to be. The breast still gets the stimulation that maintains supply, which a bottle of pre-mixed liquid does not provide. And when something goes wrong — a rash, a change in stools, unusual fussiness — you can tell which input changed, because they were never combined in the first place.

When mixing is the better call anyway
Keeping them separate is a default, not a rule. Mixing earns its place when:
- A baby refuses formula outright. Starting with mostly breast milk and shifting the ratio over several days is a standard way through it, and it only works in one bottle.
- Someone else is doing the feed and one bottle is the difference between a parent sleeping and not sleeping.
- You are weaning off the breast and the gradual ratio change is the point.
None of these are compromises on safety. They are trade-offs against waste and against troubleshooting clarity, and a tired household is entitled to make them.
What mixing does not do
Three things it gets credited with and does not deliver:
- It does not make the formula better. The breast milk's live components are not distributed through the formula in some upgrading way; you have a bottle containing both, and each does what it does.
- It does not protect against contamination. Breast milk has antimicrobial properties, but they do not sterilise a bottle or extend how long it can sit out. The clock above is the clock.
